At Insight Hand Therapy, we take your privacy very seriously. We are committed to protecting your personal information and providing a safe and secure environment for all of our clients, as set out in the Privacy Act 1988 (Privacy Act).
Collection of Personal Information
We collect personal information from our clients in order to provide quality Hand Therapy and Occupational Therapy services. This information is collected through a variety of methods including face-to-face interactions, phone calls, electronic interactions (such as email) and referrals. We gather information from a variety of other sources such as other health professionals (for example your referring GP), funding bodies or your guardian (if relevant). We only collect information that is necessary to provide our services.
The type of information we collect includes (but is not limited to):
Name, date of birth, addresses, contact details.
Health information including medical history, medications, social history, and your connection with other health providers.
Information regarding your body function and structures, activities, participation in activities, environment and personal factors.
Medicare number, National Disability Insurance Scheme (NDIS) number, My Aged Care number, or other government agency number for identification and claiming purposes.
Health fund details.
Use and Disclosure of Personal Information to Third Parties
Insight Hand Therapy uses your personal information to safely deliver quality Hand Therapy and Occupational Therapy services. We also use this information to communication with you as needed in relation to your health care and therapy appointments. We will also use your personal information for administrative purposes, such as billing and invoicing. We take all necessary steps to ensure that your personal information is protected from unauthorised access, use, or disclosure.
Your personal information is stored by Insight Hand Therapy in electronic records in secure cloud-based software. Any hard copy paperwork is scanned to your electronic record and the physical paperwork is stored securely until destroyed (shredded).
We will never disclose your personal information to any unauthorised third party, except in cases where we are required by law to do so. We share your aspects of your personal information (with your consent) with trusted third parties to ensure effective service provision – this is further explained in our consent form which will be provided to you prior to your first appointment with us. Examples of trusted third parties may include (but are not limited to) your GP, other involved health care providers, health fund, WorkCover QLD etc.
Access to Personal Information
You have the right to request access to your personal information and to make corrections if necessary. We will take reasonable steps to ensure that your personal information is accurate and up-to-date. If you wish to request a copy of your personal information, update, or correct your personal information, please contact us. We may recover from you our reasonable costs of supplying you with access to the information we hold about you. Your request to provide access to this information will be dealt with in a reasonable time.
If there is any exceptional circumstance in which we are unable to provide you with access to the information we hold about you (we will be guided by the National Privacy Principles) we will provide you with reasons for the refusal.
Changes to our Privacy Policy
We reserve the right to update our privacy policy at any time as our business requirements change, to reflect feedback or law changes.
